Terms:
There will be one 1st prize winner. The winner will be picked on May 31st, 2025. The dental office picked must be willing to accept new patients otherwise you may need to change locations of dental office. Winner must follow all rules of dental office locations. Winner is only getting maintenance treatment which includes exam, cleaning, and emergency exam. Winner is responsible for all additional treatment/maintenance/emergency not included in the Dental Membership Plan.
i.e. fillings, crowns, root canals, retainers, nightguards, bleaching, etc.
